How do I get an OPUS card for those aged 65 and over?
Online: Fill out an application form for an OPUS card with photo, provide photo ID and pay the card issuance fee ($15).
In person: Visit one of our service outlets to have your application processed and obtain your card. Be sure to bring photo ID.
